1) Who do you want to take your survey? What age, what country, etc?

2) What are you going to do with the information?

You say the target is youth, but your questions don't seem to be directed at young people.

Some of your choices are "this" or "that" - will the person answering be able to check all that apply?

Then can I suggest that you break down the age group a little more for the younger group (there will be a difference, I expect, between an 11 year old and an 18 year old), and once you get to the upper end of what you consider "youth," that you simply say "X or older."

Do you expect that youth will be married? Have had time to have more than three children?

Cable is another method of connecting to the Internet.

If the person does have a job, you might want to specify that you want to know about their Internet use outside of work.

A question you might want to ask is whether they have to share the computer they use with other household members.

And all of Clive's comments too, of course.

I still question your need for age groups going so high (and the number of children the person has) as useful for a study that is intended to be for youth.
