A health insurance contract is acted upon after ensuring the payment capability of an individual or a firm and the amount covered is decided after estimating the comprehensive cost of risk for medical care, and a periodic payment structure is agreed upon, which may be either in the form of a premium to be paid monthly or annually for private insurance companies, or annual tax if the health cover is in the form of a social welfare.

Please advice whether this sentence makes sense. Also, kindly comment on the punctuations.

The sentence would be much easier to understand if it were not so long. Do you have to make one single sentence? Corrections: 1) is acted upon- do you mean "is established"?

Corrections:
1) is acted upon- do you mean "is established"? When you act upon something, it is usually already in place.

2) an annual tax if the health coverage is in the form of a social welfare.

Should I replace acted upon with 'established'?
If you are talking about setting up a new insurance contract, yes. If you are talking about paying a benefit or premium, or any other activity related to an existing contract, then "acted upon" is OK.
