Hello, just wondering, does young or fresh faced work better in this sentence?

He lay in the gutter for hours before he was stirred by a young/fresh faced woman who possessed a remarkable likeness to Johanna – only she appeared to be in her early 20s, having lost the childlike innocence which, to him, made Johanna beautiful.

Yes it would, especially given that you mention she's 20 years old later in that sentence.
Remember to hyphenate it though (fresh-faced woman).
