Years have been kind ...

Hi

If somebody says, upon meeting someone after a long time: It's been a long time, John. I can't say the years have been kind to you.

Does it mean he knows that he had a hard time for the last few years?

It generally means "You look like ****." It's not a very nice thing to say to someone.

Hi

But that person answers: They've been better to me than they have been to you.

So is he replying that the other guy also looks like ****?
