Writing email to Employee Relations requesting claim status

Hello,

I am in doubt how to write an email requesting status of my claim. I need is to remind Employee Relations that it passed longer than 9 months from the moment I requested back pay for certain period. My contract was for job A but I worked 5 months on position B (more money and responsibility). Therefore, according US law and Company policy I have right to get back pay. This is just for you to have better picture of my situation. I provided enough documents to support my request.

Email should:
- remind ER representative
- help me obtain current status of my claim (I really need to get this so I can decide to go on higher level or to give up)
- be used as a proof if I move to higher level (won't say that in email)
- to explain how patient I was and how much I appreciate their work Emotion: smile but can't wait any longer Emotion: sad

So, please help me out - need just few sentences. Thank you in advance.
