Writing an invitation letter...

Hi my name is Melissa, I have a friend that lives in Baku, Azerbaijan. I want to invite him to come to visit me in California(U.S.A) for 2 weeks. I have some questions on how to do this. I know he has to get a B-2 vistors visa and passport, but my question is how do I go about writting the invitation letter to him? Is there a document I should go by?, or can I just write the letter? Do I need anyone to ok the letter before I send it? What information do I have to included in the letter? How do I send him the letter? i.e. can I send it via e-mail, fax it to him, or send it by mail? Also is there a deadline when I should send the letter to my friend? For example if I send him the invitation letter and he gets it in December and he gets his visa, can he come visit me in May of the next year? So if someone could let me know how to go about this process step by step it would help me out alot. Thank You...Melissa
