Writing an email to ex-boss for a prospective opportunity in his organisation

Pls help me in writing an email to my ex-boss, for a prospective opportunity in his organisation near my place. i need to thank him for everything n at the same time indirectly ask him to consider me for the opportunity. how do i do that?
