Would you please help me to find a suitable phrase?

Hi, I want to submit a research proposal for admission consideration to a university. I'd like to use a suitable phrase in the first page of the research proposal. However, I'm not sure how I should write it in order to seem formal. Would you please help me? Is the following phrase suitable?

"Submitted to the Graduate School of The University of X for admission consideration to the XX PhD program of fall 2015"


or would you please suggest any other phrase?

Isn't it better to use "graduate school of the university of XX" rather than "the university of X graduate school"? I'm not sure which one is better, but the first on
