Would rather / had better

I have just taken a test in English. There was an exercise.

"Why lie to him about your condition? You .......... tell him the truth or things might get worse."

I had to fill in gaps with either "had better" or "would rather" (There were also "should better" and "would better", but they are clearly mistakes.)

What do you think? Which one is correct?
