Would like, how you felt, the care

We would like to know how you felt about your experience in our A & E department.
Please fill in the feedback form- your comments will help us to improve the care we provide.
Please explain to me the grammar of "would like to" , the grammatical function of "how" and "the" before care?
Is " how you felt" a nominative clause to object "know"?
