I'm not sure what a earning development work sheet is, so I'm not sure if this is entirely correct.

I am writing regarding my 2006 earning development worksheet. I received my statement, and it indicated "no earnings," which is incorrect. I have called the telephone number provided on the worksheet, and they filed ?? a corrected form?? ?? the form to correct the earnings ?? for me and told me that it is now in your computer system. I was referred to your departament so you can an start investigation on my case.

But I'm a little confused - "they" completed a form and this form is now in this new department's computer system. Do you want them to do something with this form to get you a revised worksheet? Or do you want them to investigate? What do you want them to DO?

I want them to investigate on my case. To do so they need "the form" that was completeted during my phone conversation. That's what I've been told. Thank you again for your help.
