Working in an office and i want to write my MBA exam. Suggest me how to write a leave application letter.

Suggest me is this format is correct or not.

This is to inform you that, my MBA exam falls on may 10th and ends on may 15th. As i need to attend the exams, kindly provide me five days of leave from may 10th to 15th.
