Wordings for a commemorative Plaque ???

Hello,

One of my colleague asks me to check the sentences on his boss's commemorative Plaque below,

" 20th Anniversary of joining in XXX Commemorative Plaque

We are giving you this Commemorative Plaque with respect and gratitude for the dedication and enthusiasm you shown with a constant mind for the development of the company and the team members during the past two decades."

At google search I found out that certain sentences like "We here by present... or presented to" are repeatedly used. Since I am not a native English speaker, I have no idea if those sentences are proper or not on the plaque.

Could you guys help me out?

Thanks.
