Wording help please?

Could you please check these sentences for me?


Does it include a tax?

No, it [ does / is ] not included a tax. (Is this sentence a passive form?)

Drug and alcohol may [ jam / scramble ] our brain, therefore we are better to avoid them. (Both of these words are pretty much identical in the meaning?)

Passengers [ jammed / crammed ] in the bus, I was so [ jammed / crammed ] in the bus. (Both of these words are correct and common in use?)
