Why is “How” used in this statement?

So I came across this video: “How mass shootings happen.” (This is a sensitive topic so I’ll be abbreviating it to ms)

At first, I thought it would show ms, because it said “how”, right? Instead, it showed police corruption and how the police basically “let” someone do it (it was a skit - basically a white person was obviously going to do it, but a police was still oblivious to him.) However this is not “how” it happens, right? “How” it happens would show the way that it happens, but it didn’t show that, does “how” have another usage or something?

How has many uses. Basically "How did that happen?" is asking about the circumstances and the causes of the event.
