It is because you don't remember what you've read or you don't have someone to guide you. Did you start reading simple grammar books or did you go for the thickest grammar books? If so, you have the wrong approach. Start reading simple grammar books and then go for the advanced grammar books. If you have a tutor to guide, that would be better.

Did you read the books once or more than once. You must read each book at least twice to make sure you remember what you have read. Did you do the exercises in the books? If you did not, you are not trying your best.

Do you have a good dictionary? Nowadays, most dictionaries come with example sentences. You have to read the sentences so that you understand how to use the words.

I don't usually learn grammar structure ( eg : s+v+....) , I read book " Grammar in use" by Ramond Murphy .

You have to do more than just read about something to learn to do it or use it.

I think we've all had the same experience. Something seems clear when we read about it, but when we come to do it, we can't. It's because just reading something (even if you understand it) isn't really a learning process. You have to put it into use as well and practise as much as possible. I could read a book on repairing a car engine and it might make perfect sense, but I wouldn't expect to be able to repair my car next time it broke down.
