Why else wouldn’t he call?

This sentence makes me very strange. If this was a correct sentence, I think the word "else" is a adverbial here. But, what's the exact meaning of this sentence?

It means "can you suggest any other reason he would not call?" - with the sense it will be hard to find an alternative reason for his not calling.
Presumably they were waiting for a phone call that never came.
