Why

Hello,
A dependant why-clause is used as a statement heading:

1. Why you should have a medical examination once a year

No full stop has been used in my book.

Is the above sentence correct?

Would you please suggest a context in which the above sentence used?

Thanks.

The structure is common in titles and headlines. It isn't meant to be a question. What follows the headline gives the reason why what the headline suggests is true.

As a comment:
To me, the book doesn't want to ask a question, in fact. It wants apparently to explain the reason why we should have a medical examination once a year.
The heading/title statements don't have a full stop at the end usually.
