Whole vs in whole

"The tooth needs to be extracted in whole / whole"

Is there any difference between these two?

"in whole" is not correct there. "whole" is OK, but since this is, as far as I know, the normal way in which teeth are extracted, I wonder whether it is worth saying. By saying "whole", what are you contrasting with?
