Hello,
Are the following sentences all correct? If no, please fix them.
1. Who made this? (correct)
2. Who did make this? (incorrect)
3. Did you make this? ( correct)

or who made this? (Isn't this the same as 1 except that "w" is in lower case?)

That is grammatically correct as an emphatic question. It is quite rarely used, though. Here is a conversation. 

A: Who made this ingenious widget? It's so clever!
B: I did.
A: You've got to be kidding me! You are all thumbs. You can't make anything.  Now answer the question. Who did make this, really?
