The simple answer is use "I don't know who's who" when talking about people and "I don't know which is which" when talking about objects. However, "which is which" can sometimes also be used for people, especially if you know the identities but you just don't know which people they apply to. For example, you know it's a picture of Tim and Ted, but you don't know which is Tim and which is Ted.
