Who is it? A cat or maybe Nestor?

1) If “it” can only stand in for an object or an animal (according to grammar and the Nestor's answer to me), then why we say when somebody knocks the door: Who is it? (Maybe do we suppose that an animal is knocking the door?
2) Why we say: "Oh, it's you!"
3) OK, “of unknown sex”. Does it mean I can say, for example: "He (I know my cat is a male animal) loves my sister Patty"?

It's (IT!!!) just a joke with you (YOU) Nestor. Don't be mad with me. Seriously I have these doubts!!
