Who has vs with

This is the most confusing thing I have come across and even native English speaker have given different answers. What do you think (CJ Emotion: wink)?

Which is correct please?
I've never seen a woman who had X without Y.
I've never seen a woman with X without Y.

Which of these is natural?

1 A Rarely do you see women with artificial nails have all the nails. Most of the time, the women is missing a nail because it's fallen off.

1 B Rarely do you see women with artificial nails who have all the nails. Most of the time, the woman is missing a nail because it's fallen off.

1 C Rarely do you see women who have artificial nails with all the nails. Most of the time, the woman is missing a nail because it's fallen off.

1 D Rarely do you see women with artificial nails with all the nails. Most of the time, the woman is missing a nail because it's fallen off.

2 A Rarely do you see women with nail polish on have all the fingers nail polish. Most of the time, at least one nail nail polish is chipped.

Is 'who' optional?

Who is optional is this right? Does it change the meaning?
You can say 'Rarely do you see people do that'
'Rarely do you see people who do that'
