Edit. I guess this could be confusing, since "secret" can be a noun or an adjective. In this case it's an adjective.

That is, it's not like, "Please keep my secret." (Reply) "What secret?" (Answer) "My affliction secret." "secret" is

We're talking about a group of people who all have the same affliction. IMHO you may use either singular or plural in this case.
