To be technically correct, you should, in my opinion, use "are". However, native speakers do use "is" in situations like this, and "is" does not, for me, leap out as a glaring error.

if "attached" thing is not plural, you have to use "is" , "are" is used for plural.

It should be 'are'. It is pural. There are two things attached. The request is one and the reciprocity policy is the other.

To use 'is' here would be a glaring error to me.
