Which phrase is most appropriate at the end of an official letter?

When I write a official letter to my boss, we end the letter with the phrase 'Yours faithfully'. Which phrase I should use to end a formal/official letter written by me to the manager of my bank where I am the customer?

Hi, It used to be customary to end as follows. Dear Sir or Madam, (ie you do not know the person;s name) Yours faithfully, xxxxxx or Dear Mr. Smith, (ie you know the person's name) Yours sincerely, xxxxxxxx But customs change.
