Which one is formal & efficient?

Hello everyone, Please help me as following,

1) I don't understand.
2) I have not understood.

In conversation, Which one should I use?

And I don't know the proper use of "as following"

If you are talking about something that you don't understand right now in conversation say, " I don't understand."

If you are talking about something in the past, you can say "I have never understood...". For example: " I have never understood Einstein's theory of relativity."
