Which one is correct

(1) He insisted that the accident had happened yesterday. (2) He insisted that the accident happened yesterday. Which one is correct? I am confused with Answer (1) because I assume past perfect cannot go with yesterday the time concept. Or maybe not. It can go with any time concept? Could you help me clarify it? (3) He insisted that the accident should happen yesterday. I really don't understand the meaning of the sentence. Could you paraphrase it for me.

Both are usable. The first is probably not used as often because the coincidence of events that it describes
