Repair of defective door to guest bathroom / repair of defective door in the guest bathroom / repair of defective guest bathroom door

Hi, They are all OK. I'd say #3 would be the most common when writing what seems to be a point-form list. Unless you worry that the reader might think you are writing about a bathroom for defective guests.
