Which of these three sentences are correct?

Hello,

I prioritize based on what is the most important thing that i do, or
I prioritize based on what the most important thing is that I do, or
I prioritize based on what the most important thing that I do is.

I personally would believe the last sentence is correct, but the position of the verb "is" too far away from the question word "what."

Please clarify and explain further.

To me, these all sound rather awkward.
I'd reword as
eg I prioritize based on the test score.
eg I prioritize based on the importance of the things I have to do.
