I totally forgot how she annoyed us. /what annoyed us about her. Why were we annoyed with her?

I forgot how she annoyed us- could mean I forgot what she did to annoy us or I forgot how annoying she was.
I forgot what annoyed us about her- Fine.
I forgot why were we annoyed with her- Does not make sense as is. Why were we annoyed with her? or I forgot why we were annoyed with her
