Which is grammatically correct?

1. Did you tell him that you are going to quit?
2. Did you tell him that you were going to quit?

Actually the thing is yet to happen in future.

1. Had he not followed his guidelines he would be dead by now,
2. Had he not followed his guidelines he would have been dead by now.
