Because the address & phone number a group of similar material I think you should use "is". If you split the sentence you would have "Our new address is xxxx. Our new phone number is ***".

Yes this is correct. Two subjects joined by the conjunction and are plural and therefore require a plural verb.
ex: French and Italian are both Romance languages.
If you see the word "and" and between two subjects, the verb you use must be plural as well.
