Which is correct? Are they both correct?

1) The surprise was he did miss once, from 40 yards.

2) The surprise was he did miss once from 40 yards.

This is for an article I'm working on. Thanks.

1 - He missed only one time, ever, and that happened to be from 40 yards. 2 - He may or may not have missed other times, but from 40 yards, he missed only once. ")

1 - He missed only one time, ever, and that happened to be from 40 yards.

2 - He may or may not have missed other times, but from 40 yards, he missed only once.

(I would put a "that" after "was.")
