Which article

I called an ambulance and after a while a(the?) doctor gave her first aid.

Both are possible. According to the general rule, you would use "the" if the doctor had previously been mentioned or was otherwise already known to the reader or listener. If not then you would use "a".

A couple more comments.

'First aid' is usually preliminary help given at the scene, eg of an accident. We usually don't speak of a doctor giving first aid, since they have the ability to give more comprehensive treatment.

Where I live, a doctor does not come with an ambulance.
