When reading a book........

I've started reading books to improve my English...

There were vocabularies which I don't know, so I had to look up those words in dictionary, and it takes so much time to read book and find words in dictionary..

My question is ...

when you read a book do you guys look up every word that you don't know in dictionary?

You should vary your procedure. Sometimes look up all the words you don't know. Sometimes try to understand as much as you can without using your dictionary at all.

When you do look up a word, read all the definitions. Practice the pronunciation. Write it in your vocabulary notebook, which you review regularly.
