When I submitted/forwarded my application on November 11, 2008 I failed to submit some of the documents and also forgot to sign the cover letter because that day my grandmother was gravely ill and I was rather in hurry. Please include the given documents in my file. Sorry for the inconvenience.

To whom it may concern: <<<< Don't say this. say 'Dear Sir', or 'Dear Mr. Smith' if you know the name.
