When do I need to put an article before an object?

When do I need to put an article before an object?
I am quite confused!

For example:
A) I have an apple.
which means I have (one) apple indeed.

but:
B) I have came to school.
why we don't need to put a "a" before school?
and I know if that school has been mentioned before, I need to put a "the" before the school.

THanks
