What's with the hole?

Matt is trying out a second-hand suit, and there is a hole on the chest. So Matt asks the tailor (or the clerk), "What's with the hole?" And the tailor replies, "That's where the bullet killed him."

My problem is what does "what's with the hole" mean exactly? Does it mean "What's the problem with the hole" or "What's the purpose of the hole?" Thanks.
