What's the wrong with this sentence?

This sentence supposed to be wronge as my teacher said to me :

The process by which the decisions were made it was very slow.

What's the wrong with it?

You have two subjects.

Highlight the entire subject. Highlight the entire predicate. Do you see a word left over? A small word -- but it makes the sentence ungrammatical. Do you see it?

It should be: the process by which the decisions were made was very slow. Leave off the "it" after "made".
