What's the question to this answer?

Answer: I work in construction?

Can you say: "What do you work in?"/ "What domain do you work in?"

Answer: I work as a doctor.

Is this how you'd say it?

"What do you work as?"

We normally just ask 'What do you do?'

and you never say "What do you work in?"

Is it natural or grammatical?

You could say, "What field do you work in?" but usually you just say "What do you do?" as Clive said.
