What's the meaning

of tears at me?

"Once I graduated, we starting falling apart. We are still decent friends, but nothing like we were then. But I keep coming back to him in my mind. Nothing I do gets me away from it, and it tears at me."

So I guess it has two meanings?

I also found another idiom that's similar "tear at your heart", but I guess this could be different than the one the author is using.
