Is this okay as written? What would you change (add/delete) in terms of grammar or punctuation?

On Thursday, 9 May 2004 at 1656 hours, Joe Smith (Human Resources Director) informed me, Jane Doe (Vice President of Marketing for XYZ Corporation), of an incident involving alleged sexual harassment.
