What were the main stages of development of English according to Henry Smith?

Not so long ago I had a test concerning the history of the English language. In the test there was a question:" What were the main stages of development of English according to Henry Smith?"

Of course I didn't know the answer to the question. After the test I even searched the web but couldn't find a thing. If you know the answer to this question please help.
