What went wrong?

"What went wrong?," he said after the speaking engagement.

I see he didn't think his speech went very well.
As another interpretation, could "What went wrong?" mean "Couldn't be better!", where he's sure it went very well?

I think your second interpretation is unlikely; sounds to me as though the chap was lacking self-confidence in his performance (or, indeed, that he knew that he'd spoken badly).

If, however, he'd said "How good was that!' that probably would be a sign of an (overly-)confident individual!
