What to put in an official form?

I am translating a form used by outreach youth workers and in the form there needs to be a bit about a minor's guardian's information. Do I simply put:


"A minor's guardian's information"


or do i put:


"Minor's guardian's information"


or possibly:


Guardian's information (in case of a minor)?
