what is wrong with this sentence?

The Performance Appraisal Report submitted for the period 01.01.06 to 31.12.06 indicates that Ms. Phillips’s job performance and conduct had been assessed as ‘Outstanding’.

The grammar is correct, but without any context I see no reason to use Past Perfect.
I'd just say '. . . was assessed. . . '.
