What is the difference between "noun + be + past participle" and "noun + past particple"?

Hi all!
I'm wondering why in some cases people skip "be" in passive sentences? For example, if I see "404 error" on any web-site, it says "page not found". Why it's not "page is not found"?
What is the difference between these 2 forms?
