what is the correct Ans?

The deadline dates for the construction ______ by the architect.

(A) have changed
(B) was changed
(C) have been changed
(D) was being changed

I think it’s D.

New safety requirements ___________ by the government.

1. Have created
2. will Create
3. would create
4. were Created

I think it’s 4.

but both the question has similar structure. Hence if 2nd one is 4, then why not the 1st one is B.

I am getting confused.

for the 1st question, if "were changed" and "have been changed" both were present in the options then which one would have been correct?

2ndly if only "were changed" present and not "have been changed" present then, should we consider "were changed" as solution?

Both are correct grammatically. The choice of the tense would depend on other factors. I don't understand your second question.
