What is right meaning?

Heading of a news "Teenager stabbed to death", what is it mean?

1. A teenager stabbed someone to death.
2. A teenager is stabbed by someone.

which one is correct?

According to news report "A teenager was stabbed to death by a group of young man"

Even without the article itself, the headline can only mean that a teenager was stabbed to death by someone. If the teeneager had done the stabbing, the headline would have been something like: Teenager stabs man. / Mann stabbed by teen.

A person / thing does something (Present simple)
Example : Astronauts arrive to the Earth from space station.
something is done with a person / thing (present simple passive)
Example :Two person killed in an accident.
